Short answer, patience and enabling clipping and a willingness to spend a lot of time nudging things slightly in advanced mode to get them just right. Reference images are a big plus for it as well. I don't have anything I've built to that scale yet but I am always amazed with these.
It’s also crazy how people design this stuff when there isn’t any functionality for selecting/moving several items together, or treating them as groups. Like if the placement in the room is off then every sub piece needs to be moved individually.

It clearly has less keys than a normal piano, so it must be to accommodate less fingers on a tauren.
Though I guess this means it's not compatible with a regular piano and needs its own music lol.

From the screenshot it looks like belemath planks and platforms, the silvermoon rectangular planks, along with some dye.
For this one, and a lot of the ones you will see, its all about clipping into other parts of the design and turning things to different angles.
I think /r/wowhousing is a sub. Or something to that effect.
This is truly such a creative way to use the housing decor.
