It's capitalizing on the controversy regarding the Sumud Flotilla, there's the accusation that the Italian opposition is trying to focus the spotlight on the Italian partecipants to it solely to "cause trouble to the government"; and the cartoonist here coupled it with some comments made by a semi-famous youtuber who used to be famous for largely being unbiased, saying that protesting against Israel using specific slogans (specifically "From River to The Sea") was "bad optics", which was taken really badly by ProPal people.
It's basically accusing Meloni (and supporters) of either paranoia or whataboutism.
