The issue isn't that people express themselves through close up gooch-shots in their portraits. The issue is that portrait is then imposed upon others. Sexual content / nudity imposed without consent is a big no-no, legally and ethically. (No nudity in the game of course, just including as an example.)
That's why games have ratings and tell you in clear terms why it's rated that. So, in business terms, that makes SE liable if any content whatsoever violates the established "contract." Full-stop... it's also just basic consideration. Not everyone wants a crotch shoved in their face, from virtual screen or otherwise. Ratings give the heads-up as to whether crotch-in-face is actually a certain encounter, so players can make an informed decision about whether they want to engage or not.
(It's also why things like streaking or exposing one's self in public are considered crimes, whereas nude beaches are not. Nude beaches exist upon an unspoken contract. "If you come to this beach, expect nudity.")
I personally tend to laugh or chime in with the usual "jfc XD" when faced with these more egregious "expressions," but just because I'm unbothered doesn't mean everyone else is, or that established ratings mean nothing.

I'm increasingly frustrated with this idea that we need to coddle and deprive everyone from the adversity of reality. This sort of suicidal empathy crusade that has commandeered the world.
I'm not suggesting the world needs to mold to my ideal, but just that maybe we should consider that all forms of expression actually have value and represent a form of our transcendant human experience.
I believe that the discomfort of being exposed to expression people may not want to be exposed to is actually an adversity that is crucial to human thriving. Offensive, unwanted, or shocking expression is not just tolerable but essential for intellectual and personal growth. That to overshield, erodes personal liberty and resilience leading to societal sanitization and homogenization.
I think that the very idea that sexual, suggestive, or nudity based content makes people uncomfortable or is seen as a taboo in certain contexts is actually revealing of deep conceptual and philosphical problems in society and it's worth confronting.
