Another suspicious thing was that 6 tetras hadn't cured it, but then I remembered they've been a bit broken for a year now. The regular old 4 pills max isn't enough at the moment.
My guess is he started taking tetracycline immediately, when that wouldn't cure it. The missing health tick is from when it finally went stage 2 and was curable by the tetra
Unless the mods you're using have changed that, according to the screenshot, you don't have a stage 2 wound infection.

The vanilla infection would have dropped your health a lot more. Also vitamins don't affect it.

u need iodine, desinfektion spray or alcohol tincture.

Multis won’t do anything for wound infection, and the longer your on stage 2 the more tetra you need. And it’s usually a lot.
I read somewhere that multi+tetra is a boost, that proves it wrong now. I needed 6 to fully heal, that was a trauma lol
Multi does not help wound infection at all.
