You should speak to your university, they will give you the address to do the medical check. There is a specific place you need to do it, if you get it somewhere else you will still have to pay for getting it verified at the address you need to go to.

You do the RP within 30 days of entering the country (I'm assuming you're on X1 visa because for X2 these are not a thing), most of these docs are from your uni, yes. Medical as someone else said will be an appointed facility, your school will tell you. Accommodation reg form is the paper slip you get once you do the police office accommodation registration within 24hrs of entering the country. When I did it my uni also gave me a stamped paper that confirms enrollment and says "this person is enrolled through date XYZ" that was needed.

Don't stress over this, your school's international office will help you out, you have ample time to do the RP and most documents you already have, remaining ones you can easily obtain. Form is filled out on-site, no need to worry about that.
