For one second I thought that second head was a kind of smear frame/movement indicator and that the guy was frantically looking at a, presumably male, viewer urging him to look at the magazine and divert his attention from his junk
I went a different way. I thought the other head was meant to represent his consciousness greying out as if he has grown bored of jacking off and is doing it on auto pilot

A guy with 2 heads beating off to a magazine while on the toilet with the caption "Hey pay attention" is what I'm seeing. Its possible that he has 2 heads, or it implies movement of the subject.

One head controls each half of the body. One is holding the magazine, the other is wanking. One isn't really in the mood.
“Blow air out of my nose” means, “it gave me a chuckle.” You didn’t laugh out loud, but you chuckled and you blew air out your nose.
