I’ve been on HRT for two years, and for a long time I’ve had yellow-white discharge in my underwear (my T level is around 15 ng/dL). My urine also smelled kind of weird. I went to the doctor thinking it might be an infection, and they did blood and urine tests, but everything came back normal. Even so, under the doctor’s supervision I got three intramuscular antibiotic shots. Before that, I had already taken two strong antibiotic pills, but nothing changed and it’s still happening.

When I told my family, they kind of brushed it off and said that even with hormones, I could still be having a male puberty-type discharge. I also hadn’t masturbated for almost 1.5–2 months. Yesterday I did, and it ended with pain that felt like it was coming from inside my urethra, like something was poking or stabbing from the inside. I thought it might be from atrophy, but today I found almost a teaspoon of blood in my underwear.

I’m planning to tell my family and go back to the doctor, but I’m really nervous that they’ll just blame it on hormones. Have you ever experienced anything like this? Between the constant discharge and now the bleeding, I feel pretty helpless.

    It's also not a common side effect of HRT.

    You should go to the doctor ASAP.

    Also, don't tell your family - if you are an adult, you don't have to.

  • Go to a hospital don’t wait this could end up with a uti or infection I don’t know you but don’t want you hurt btw I’m not transgender but my mom is a nurse and I grew up learning about a lot of this stuff

  • I would ask to see a urologist and try to get your thing ultrasounded. Blood in anything that comes out of it is serious.

    Also antibiotic injections and pills sounds like overkill. The blood might be a reaction to the antibiotics, but you're really going to have to be insistent and persistent to get the answers and treatment you need. I know it's a cliche but it's one more "welcome to being a woman" instance.

    Not a medical professional but I have some chronic conditions and very much take charge of my own physical health so I've had to do my due diligence. This is concerning. there might be a simple explanation but you can't risk it being not simple.

  • A teaspoon of spontaneous blood and internal stabbing pain are absolutely not normal side effects of HRT, so please do not let your family minimize this as "puberty" discharge. You need to see a urologist specifically not just a GP because this sounds like it could be a prostate issue, a urethral stricture, or a deep infection that standard urine cultures missed. Atrophy can cause minor discomfort or spotting, but it definitely does not cause that volume of bleeding; get to a specialist immediately.

    That condition (basically a very mild intersex condition) would go undetected in cis men because hormone levels don't allow that tissue to develop.

    The most concerning possibilities are an infection that has escaped antibiotic treatment (more likely) or cancer (less likely). So a doctor shouldn't just brush things off - I would if an ultrasound and biopsy is a good idea. I'd share that case-study.

    Is there any possibility of prostate issues being the cause im worried about them looking in one place when it could be another, and does a proctologist handle prostate issues or urologist.

  • Go to a urologist. If it's not an infection it's possibly cancer.

  • THAT IS NOT NOTHING. GO TO A DOCTOR. And if your family would give you such a useless answer don't tell them. You're an adult. Bleeding is not a side effect of HRT.

    I see everyone here has already thrown possibilities at you, in my experience this sounds incredibly similar to a kidney stone. They can mimic the symptoms of a UTI until they actually cause one, and the pain often lies dormant until suddenly it isn't. The smelling urine and blood are signs of either UTI or trauma, both of which are caused by stones. Again, go see a doctor, as UTIs have a million other causes, but this is certainly not normal.

  • I had this issue and it turned out to be a kidney stones. Not as much blood though. My pee burned constantly. Go to a doctor ASAP.

  • I’ve experienced something like this once but no bloody discharge. My boy part doesn’t respond like it used to. There’s a thing called “retrograde ejaculation”. Look it up. The blood is a concern tho.

  • None of this sounds normal to me, at least I have not experienced these symptoms from HRT, and bleeding in most cases is almost always bad. Did the symptoms only start after starting HRT? Or was the discharge thing even before that? I would assume infection or maybe STI if so, though I am not a doctor, but either way it doesn't sound right whatever it is. Please go to a doctor (maybe a different one this time) and get things checked, you don't want to risk dying of an infection or something. I know it's awkward but you'll have to do it. Good luck :3

  • I think it's good to get it checked with a doctor, but here's a plausible possibility:

    Maybe you had pre-ejaculatory fluid that looked a bit off (though it was really important to get it checked with a doctor as it seems to have resembled a lot some gonorrhea discharge) (keeping in mind "precum" is produced in even greater amounts when you're on HRT for a lot of people).

    As for the blood, you likely caused some amount of urethral trauma when masturbating due to the atrophy. In any case, it is a cause for concern and it's a good to get it checked by a doctor.

  • I had something very similar to this happen to me. It ended up being a very small tear in my urethra. Basically there was some atrophy of the tissue stretch ability and a full erection stretched it too much. Does urinating cause any discomfort? It would rarely be painful to pee when I has having the issue but was often weird feeling.

  • Bad sign, you need a doctor. I can’t think of a hormonal side effect that would cause this

  • I am post-op and I do see blood in my panties or in the toilet bowl occasionally, but none of it was in my urine - some bleeding in my vaginal canal occurs (way rarer and less now, months into recovery) and I also have to deal with hemorrhoids every now and then. But you know what doesn't have blood in it unless something is very wrong? My urine. Never actually seen that happen before, and would immediately be getting to an emergency room and screaming "i am pissing blood" if I noticed blood in my urine.

    The only time I've ever personly seen blood in a mammal's urine was when I discovered one of my parents' Farm Cats was peeing blood in their litter box. She also had been lethargic and showed some other symptoms that my parents had been ignoring, but the bleeding was what made them finally (with my urging) take this half-wild outdoor cat to the vet. And the vet immediately did some scans and told us that the poor kitty had Kidney infection and was in the late stages of Renal failure - her kidneys were shutting down. She did not make it.
