I assume it's also for if you try to target an enemy/part but a faster unit takes them out first and cuts off Faust's attack.
That or so that turns where enemies with huge attacks that ends the turn just don't end up giving you one stack when you can't do anything about it

From what we know, no, they don't You could in theory, as a member of Index, randomly get murdered by another member, because prescript said so. It's all according to the will of the city and what's best for the Index as a group, not you as an individual.
the prescript is vague enough that you don't need to hit them while alive only, just hit them with a skill whenever but danteh cant target corpses in gameplay so we got this
