I'm sure it's one of the most common ways that rock climbers die. I've read lots of accident reports on https://publications.americanalpineclub.org/ and it comes up time and time again.
Very easily prevented by simply tying a knot in the end of the rope.
Cleaned my rope a couple days ago and forgot to put a stopper until I was already 100+ft. Realized mid climb thankfully, but it slowed me down quite a bit. Makes you start to second guess yourself
What exactly do you mean by "this shit"? Dying because of not tying a knot? Or demonstrating the dangers of not tying a knot in a controlled environment?
Rapelling accidents are somewhat the airplane crashes of climbing - they don't happen above-average often, but they often result in severe or fatal injuries.
Tie your end knots. Use a backup prusik. Don't rely entirely on technology ("I have a Grigri, nothing can happen!").

Yeah someone just died a few weeks ago in Yosemite because they didn’t tie a knot and just slid off the end of their rope
Was that the one where dude literally ran down his rope thinking he had way more of it then he did? Man, that was just heartbreaking to see.
Yeah, his bag that was trailing behind him got caught/stuck on something. He had already made it to the top and was going back down to retrieve it.
