I was curious what it meant when it shows an activity “played for the first time” but doesn’t show the game title. Does it definitely mean they privated the game, or could it mean something else? I thought that when you private a game, it’s supposed to not show these activity logs in the first place.

There are games that are blacklisted in some countries. (e.g. nudity)
So there is a game called Space Wars and it's hidden on Steam and can't be seen by anyone and you can't find it on the store. But it exists and I have played it. You have to run a command to claim the game and download it. When I played the game the same message showed up in my activity. It said I "played" a game but didn't show the name of the game.
same for Codname: Gordon. If it is not currently installed on your machine, there is no way to see anything about it.
So if you uninstall it, it may not show a title?
If you uninstall it it won't say anywhere that you own it and you won't see it in your Steam library.
That's crazy. Delisted games usually at least stay in your library even if the store page is deleted.
The game is still in your library, you just don't see it there. It's like invisible.
These are hidden games that are built into steam. Ex: space wars

A lot of games get delisted from steam, but the games remain in the account holders if they bought them before they were delisted, and they can still download and play them.
I personally have not seen this issue myself. But I would bet it might have something to do with delisted games that people still play.
I don't think that is what's happening here. Forza Horizon 4 is delisted, but it is still there on the Steam store and it shows the name when you play it and get achievements. It just can't be bought. I think hidden games and delisted games are different.
SpaceWar is the only game like it that I know of, SpaceWar is an example application used to help developers understand the usage of the Steamworks API, and it's hidden cause it's not really an actual game. Other games that are hidden on Steam that are actual games are hidden for other reasons.
I know it's not what you meant but "not an actual game" is a bit unfair.
They were delisted...not just removed from sale, but also hidden. In most cases they kept the files on their servers so if you had paid for it before it was removed, you can still download it. A couple have been fully purged though.

If a game is marked as private, it shouldn't be generating any notifications at all that anyone else can see, that's kind of the point of marking things private.
It could be for the new FiveM thing that popped up on steam. I've seen a good handful of those pop up, including for myself, after that was introduced.

I remember years ago (like 2015 i think) playing a pirated version of the forest and it came up on steam as something along the lines of space war, didnt add anything to my library or games on my profile but did say i was playing it on my profile and tracked hours if it was within the last 2 weeks.
That and also there was a remote play exploit where you can run any unsupported game in remote play and play together. It requires using a supported game's folder to run it so even if I was playing something else, it would show that I was running cuphead, which was the game I chose.
