It does that in the series, but what it writes affects the stand and user the same way, because they are in essence the same individual. I wonder about IT since it's a lot less human. Like, it doesn't think, act, or process things like a human does. It's a primordial interdimensional force that is powered by fear, not exactly an individual guy.
In that case, I think Heaven's Door could still activate against IT, but I imagine the words in the book pages would be completely unreadable, and I don't know if Rohan could meaningfully add or remove anything.
As a disclaimer, I haven't read IT, but I've read the Dark Tower series, which features a creature very closely related to IT called Dandelo. Some think that Dandelo hatched from an egg laid by IT under the town of Derry. Regardless of its origin or direct connection to IT, this thing is 100% killable by semi-magic, semi-mundane means. So maybe Heaven's Door could affect IT, but probably not because of its nature

I feel like Dio wouldn't scare him, but grief would get to him if he turned into Kakyoin or Avdol. Pennywise playing with his head would definitely get to him. It's like in Part 6, Jolyne was his weakness. I feel like Jotaro seeing the deaths of Josuke or Koichi would weigh him down
Unironically I like the idea of an alternate part 4 where the villain isn’t a homicidal Stand User, But some kind of supernatural monster with non stand related abilities that makes it difficult for the protagonists to track down and fight even with their Stands. Kinda like Thus spoke Rohan Kishibe but with everyone. I feel like it could kind of stay true to the series’ roots of fighting the supernatural forces that isn’t just Stands, First it was vampires, Now it’s eldritch horrors.
