I think the issue is that that type of game would have an NSFW tag on it making Steam users have to opt in to see it, whereas this game probably wouldn't have any of those tags despite having fairly explicit content, and the devs disagreed with Valve's request to put it in.
My understanding is that you get one chance to get your adult title audited for inclusion on Steam. The reason is that they don’t want people gaming their rating system by constantly “feeling out what is tolerated” with resubmissions with minor tweaks. (Unlike the movie rating board which permits tons of recuts and resubmissions.)
The second that Steam sees a whiff of CSA adjacent or explicit seeming material, the game gets the permanent boot from ever being listed. This has less to do with Steam being moralistic and more that they have to keep things copacetic with Mastercard and Visa, both of whom it depends on for transaction processing.
more that, if you're trying to find the line, it means you're trying to make your game as explicit as possible without technically counting as adult content. imo, that goal in and of itself is valid grounds for your game to be labelled adult content.

Well I don't know many people who make it a habit to go and check every user setting on a regular basis, just to see if they finally added basic features. People tend to give up after years of nothing changing. I can tell you it wasn't there when both my Wife and I looked after buying our current computers in 2021, or we would have it selected.

And being able to filter out 95% of them doesn't mean that much when there are so many, and the Steam suggestion algorithms don't tend to take note of your ignoring/blocking over 100 of a certain type of game. It'll still happily try and suggest more.

No, I find it annoying that there's no function to specifically filter out those games. They acknowledge that many people want a mature filter, and there are more warnings on a lot of those "games", but they don't add a tag to them that means they all get filtered.
Say you don't like racing or sports games. If you put those into your blacklist, you'll very rarely see any of those kinds of games. You put anime or hentai or anything like that on your blacklist, and you still get at least one across a discovery queue or two. During big sales, where you get rewards for doing queues, I've still seen up to three or four in a queue, despite having those tags blocked.

That's the point of a "mature" filter, to block all mature content. The tags are community created, so they're not 100% reliable.
