Do you have Window's wallpaper slideshow on? (Where your desktop background switches every x number of minutes.) It'll cause this EXACT issue in DirectX games.
Xen is more demanding than the chapters on earth but this looks awful. 😅
I played this with a low end NVIDIA GPU some years ago and hadn't such issues.
What surprises me is that the FPS counter shows high FPS even though it looks like a slideshow.
Perhaps there is a program running in the background that is causing interference?
Honestly no idea if you're using windows it could be caused by windows eating up ram. Or some other bull crap. Or it could just be drivers. I'd recommend checking your driver's restarting the game if that doesn't work restart your PC if that doesn't work then. You could always switch to Linux. Oh uhm did you hear that who said that huh must have been the wind. But probably the most likely cause is the game Xen was added not so long ago so it's probably not that well optimized. (If switching to a second monitor solves it it could theoretically be the HDMI/DP port I've ran into weirder stuff)

Is there any chance you have your other monitor plugged into your motherboard instead of your gpu?
If so, it could be using the integrated graphics of your cpu when you switch monitors (would also explain why it runs fine on one and not the other)
Just the 3070, I ain't stupid enough to plug hdmi/dp into my mobo. Plus the 5700x doesnt even have an igpu

How is the hardware utilization?
XEN is heavy even on a very good PC specially "Interloper" chapter.
Disable any software that could be injecting an overlay. Make sure drivers are updated.
