I had routine lab work from walk in doctor this week and my eGFR came back at 75 (creatinine 82) so he called and flagged that my eGFR has slightly gone down since my 30s. Ordered an ACR which came back 1.6 and told me to follow up with GP
GP ordered another ACR today which came back 3.7. So now I have CKD?? I cried all day. I also walked 80 minutes by foot to lab before giving urine sample, so not sure if the exercise affected levels?
I also have a history of sterile pyuria, which my doc thinks is from IC. Recent kidney ultrasound normal, normal cbc and CRP. ANA is always mildly elevated but has been since my teens and I have reynaud's but no other signs or anything
Do I have CKD? Could this be the exercise or IC triggering my ACR? I am so worried I won't be able To be here for my kids
What tests can help Me get Clarity? I have no idea when my GP can see me next

Disregard, see below.
Thanks it was the urinary creatinine / albumin ratio that was 3.7. Not sure if that's the same thing as creatinine. I went on a long walk before the test, 80 mins
Ah, I misread. What was the creatinine?
Creatinine was 81 (draw with ACR 1.6) and 85 (draw with ACR 3.7). Normal range in my lab is 45-90
Ah I think I was reading US values into this. Typical creatinines here are reported in mg/dL and I'm guessing your lab's is different. If your creatinine is within the reference range twice then I'm not too excited to label this anything significant. Creatinine clearance can adjust for various benign reasons, to include dehydration, muscle use, and volume of muscle. It also fluctuates moment by moment, so if you were to repeat the same lab an hour later, it would be a few points different.
In other words, I retract my prior statement and can't say I'm particularly worried on your behalf. Stay hydrated, keep the UTIs in check, and live your life (with continued followup with your doc).
Thank you 🙏
Certainly!
